Pilots Experienced Dual Flameout Before SBN Crash
The National Transportation Safety Board’s interim factual report of the March 17, 2013 crash of a Beechcraft Premier 1A at South Bend (SBN), Ind., said both engines flamed out during the aircraft’s first approach to the airport. A restart appeared to bring back one engine just before the tower controller commanded a go-around. According to the cockpit-voice-recorder transcript, the instrument-rated private pilot, who had logged approximately 613 hours’ total time and operated the jet single-pilot, was giving instruction to the pilot in the right seat. During the first visual approach, the right-seat pilot, in an attempt to slow the airplane, pulled the throttles beyond the flight-idle position and into cutoff, flaming out both engines. A single engine restart appeared to have been successful as the aircraft descended toward the airport. On final approach however, the tower controller called for a go-around because only the nose gear appeared to be fully extended. The aircraft entered right traffic for Runway 9R but bounced numerous times on the runway following the second approach, according to witnesses. The aircraft then began a climbing right turn that ended with the crash into homes near the airport. Both occupants survived the accident but sustained serious injuries.

Вот лажа! Не совсем понятно, гидравлика запитана от двигателя, и не выходит если он не работает? И объясните мне что значит выражение The aircraft entered right traffic for Runway 9R

A small plane with three people aboard made an emergency landing in a strawberry field southeast of Santa Maria on Saturday morning.

Santa Barbara County Fire Department spokesman Capt. Dave Zaniboni said that the Piper Cherokee Arrow went down at approximately 11:45 a.m. in the field near the intersection of Clark Avenue and Telephone Road, east of Highway 101.

"The plane went down due to mechanical failure," Zaniboni said.

The pilot was treated at the scene for minor injuries, while the two passengers, the pilot's son and the son's fiance, were unhurt.

Three Santa Barbara County Fire Department engines responded, assisted by one engine from Santa Maria Fire Department and Santa Barbara County Sheriff's Department deputies.

Zaniboni said that an investigation into the incident is ongoing.

Here is the most interesting question whether it was a good idea to land with undercarriage in. Probably it was the safest way, although not without damage.

NTSB Docket Reveals Cockpit Warning Before GIV Crash
The flight crew of the Gulfstream GIV that crashed on May 31, 2014, in Bedford, Mass., received a “rudder limit advisory (blue) message” while on takeoff roll, according to more than 800 pages of data released today by the NTSB. The aircraft overrran Runway 11 after a rejected takeoff, struck lighting and an antenna assembly and came to rest in a gully before being consumed by a post-crash fire. All seven people aboard were killed.

According to the cockpit voice recorder transcript, during takeoff roll the copilot reported the rudder limit advisory message. The captain asked whether the copilot was using the rudders, to which the copilot responded, “No.” A sound similar to a power increase followed, then the copilot reported “V1” and “rotate.” Within a second the captain repeatedly reported, “Lock is on,” and then, “I can’t stop it.” The aircraft reached 165 knots on takeoff roll but did not lift off the runway.

An NTSB preliminary accident report issued in June focused on the twinjet’s control wheel mechanical gust-lock system. Study materials for the GIV instruct pilots to shut down the engines and bleed hydraulic pressure to zero if the engine is inadvertently started with the gust lock engaged.

A contract pilot who flew with the accident crew told NTSB investigators that the captain conducted complete flight control checklists before each of their flights but did not use a formal checklist. Data from a quick access recorder installed in the accident airplane revealed that out of 176 takeoffs, only two complete and 16 partial control checks were identified. There was none identified for the accident flight.

Thank you for this topic. I use English for my job.
I like learning English and I have opened that I can improve my English skills even in aviation.
But I see that this knowledge need to be improve every day. Everyone should work on our second (or third) language every day. I do it in one service that is very useful for me. I use LinguaLeo every day. I have 79 level here.
I load on this service my books or materials and then I can "drill" it.
Now I have ICAO-4. Next record should be in 3 years. It's not big time to be lazy.
